What Does a Data Capture Form Look Like?

If you have experienced filling up forms in school, hospitals, or banks, you most likely have an idea of what data capture forms look like. It is a digitized format of the paper-based forms which also varies according to the nature of the business. Data Capture Forms are designed to be easily navigated through data collection tools such as mobile phones, tablets, or digital pen and paper for seamless transaction and sharing of information.


Depending on the type of information that you need and the kind of data collection tool that you use, data capture forms can be as simple as a page with text fields and checkboxes. In the case of INKWRX Mobile Forms, aside from allocating spaces for text, it also captures data such as photographs and e-signatures for validation and security purposes. To increase the accuracy and speed of your forms, INKWRX Mobile Forms also provides what they call pre-population forms whereby it pushes existing business data to your forms.

As you can see, there are various text fields provided for each specific information. The sizes of these spaces may also vary depending on the type of data. For instance, the area for Customer Signature is almost 5 times bigger than the text boxes allotted for Type of Work, Engineer Name, Date, etc.

If you look at the third photo, the text boxes for VAT and Total are highlighted in blue. This is an example of a pre-populated form. Since VAT may be computed prior to the actual work, the data for this section can be filled even before handing it over to the client.


What Are The Other Faces Of Data Capture Forms?


INKWRX has partnered with several companies and helped them create their own data capture forms. For example, they converted Adcock Group’s forms into a digital format with digital pens as their data collection tools. These forms consist of a combined risk assessment and service sheet for reactive service, the same combination for planned preventative maintenance, a waste transfer sheet for disposal, a time sheet, and a hot works advisory sheet that’s used when engineers are braising copper pipe.


After completing a site visit, engineers now write out the relevant data on data capture forms using digital pens. Simply by ticking a box on the bottom of each form, the electronic data stored in the pen is then transmitted via Bluetooth to the engineer’s data collection tool which can be a tablet or mobile device. After a quick on-screen validation of the job number, it’s sent on to the INKWRX™ secure servers, together with any photos such as damaged equipment or parts that might be difficult to access and identify. Within less than a minute a copy of the completed handwritten form is emailed back in pdf format and is in the hands of the service centre management and administrators.
